Replace a MAST Permit

Do you need to replace your MAST permit?

To replace your lost permit, or to change your name or gender on your MAST permit, please check the MAST Permit Checker to make sure your permit is still valid. If your permit is still valid, please fill out and mail the Application to Replace MAST Permit. 

Please mail the completed application and a $5.00 check or money order made out to LCB to:

You may also contact your provider to replace your permit. Your provider may be able to accept payment by phone or online to reduce the processing time. LCB processes replacements in about two business two days from the date we get the application. If you need your permit sooner, please contact your provider.
