Change Applications 

Use these forms to make changes to an existing license.

  This application is to change the name of your business. Changes to an entity’s structure can be made using the Change in Governing People, Percentage Owned and/or Stock/Unit Ownership application.

Business Licensing Services Applications

These applications must be submitted through the Department of Revenue (DOR) via Business Licensing Services (BLS). BLS will then send the application for continued processing to the LCB. This length of this process can vary. After the application is assigned to a Licensing Specialist they will contact you to discuss the next steps in the application process.

Please note the addendum is required for all BLS applications.

Splitting a Producer and Processor License

To split your cannabis producer and processor licenses, please follow these steps:

1. Send in an application to BLS with the "Change of Location" option.
2. On the application, specify which endorsement is moving and to which location.
3. Please ensure it is clearly stated that you are splitting your privileges, and will have two locations.
4. Submit a Cannabis Alteration Form (available above) for the license that is remaining at the current location.
5. These two applications must both be approved simultaneously, to ensure both business locations are compliant at all times.
